What is Neurovascular Stent-Assisted Coiling Used For?
NSAC was a breakthrough development in the non-surgical treatment of intracranial aneurysms. NSAC can be used to address several types of aneurysms including intracranial, saccular, and wide-neck aneurysms. Aneurysms are weak spots in the blood vessels that pose a danger of bursting. In a wide neck aneurysm, the wide openings at the base traverse either side of the arteries and present the most challenging to treat. Due to the nature of wide neck aneurysms, coils alone would not suffice to divert blood flow away so extensive brain surgery was often the only alternative. NSAC allows the doctor to make a small incision in order to insert a micro-catheter and direct the catheter to the location of the aneurysm. The stent is implanted and helps the coils remain in place. The coils, in turn, provide a natural site for blood clotting and effectively deter blood from traveling to the vulnerable blood vessel walls. This system stops the aneurysm from rupturing.
What are the Risks Associated with Neurovascular Stent-Assisted Coiling?
The relatively rapid recovery time and avoidance of surgical risks has made NSAC a popular and promising procedure for patients diagnosed with aneurysms. However, in some patients, this procedure may present serious risks that outweigh the benefits. The adverse events reported in connection with the procedure have led the FDA to issue a statement advising that health care providers carefully consider patient selection and comply with proper use of the device to avoid life-threatening complications or death. NSAC may be negligently preformed on patents who either (i) have other conditions or risk factors that contraindicate the use of NSAC, or (ii) are not tolerant of the concomitant anti-coagulation therapies that are necessary following the procedure. In other cases, the device may not operate properly and result in a rupture of the aneurysm. In patients who present any of these risk factors, other more conservative measures are highly recommended.
On May 8, 2018, the Food and Drug Administration (FDA) issued a warning about the use of NSAC for wide-neck brain aneurysms and cautioned patients that they could be at a higher risk of stroke or death after undergoing this procedure. The FDA statement follows other reports highlighting the thromboembolic risks associated with NSAC procedures.
