What are PPIs?

Nexium (esomeprazole), Prilosec (omeprazole) and Protonix (pantoprazole) belong to a class of medication known as PPIs. Other PPIs include Aciphex (rabeprazole), Dexilant, Prevacid (lansoprazole), Zegerid (omeprazole; sodium bicarbonate), Vimovo (esomeprazole magnesium; naproxen), and Kapidex. Most PPIs are available in both prescription and over the counter (OTC). PPIs work by blocking an enzyme that regulates acid production. PPIs target conditions that are caused by excessive production of stomach acid and can be used to treat acid reflux (GERD), duodenal or stomach ulcers, Helicobacter pylori (H-pylori) and erosive esophagitis. PPIs are among the most prescribed medications in the United States.

What is Happening with PPIs?

Researchers have found that regular, long-term use of PPIs may be connected to a two-fold increased risk of stomach cancer. The patients presenting with the highest risk of cancer are those that have been diagnosed with an H-pylori infection. PPIs are the standard treatment for ongoing H pylori infection, which itself has been associated with later developing stomach cancer. While eradicating H pylori is thought to reduce the risk of stomach cancer, many people continue to develop stomach cancer.

Observational studies found that that the risk of gastric cancer while taking PPIs increases with higher dosage levels and longer periods of treatment. As a first-line medication to treat H pylori, PPIs are considered safe for short-term use. However, long-term use of the drugs is not recommended. Scientists believe that the suppression of gastric acid may encourage the development of gastric cancer. These findings have prompted a more restrained approach towards the use of PPIs as maintenance drugs. Other studies have also shown that PPIs increase the risk of liver and pancreatic cancers.

The World Health Organization (WHO) considers NDMA as a “probable human carcinogen,” which means that it likely causes cancer, especially after repeated exposure.
