What is Xeljanz?
Xeljanz is taken as an oral medication to treat moderate to severe forms of RA, an autoimmune disorder that causes chronic inflammation in the joints. Xeljanz can reduce pain and tenderness in the joints associated with RA. It is also used to treat psoriatic arthritis, which is a form of arthritis that affects patients with psoriasis (a skin condition), and ulcerative colitis, which causes irritation and swelling in the large intestine or colon. Xeljanz is often prescribed to patients who have not responded well to methotrexate, a drug that is initially prescribed to most patients with RA. Xeljanz was first approved by the FDA in 2012 to treat adults with RA. Xeljanz was approved to treat psoriatic arthritis in 2017 and ulcerative colitis in 2018.
What is Happening with Xeljanz?
In February, 2019, the FDA informed the public that the results of a clinical trial showed an increased risk of blood clots and death when patients with RA were on a 10 mg twice-daily regimen of the medication. This specific dosage has been approved by the FDA only for the treatment of ulcerative colitis, not RA. The currently approved dose of Xeljanz for patients with RA is 5 mg twice daily. An ongoing safety trial wherein the manufacturer transfers RA patients to the currently approved dose is scheduled to conclude at the end of this year. The European Medicines Agency (EMA) has released a similar warning to healthcare professionals and patients to avoid exceeding the recommended dose of Xeljanz for the treatment of RA.
Patients and healthcare professionals are advised to follow the recommendations for taking Xeljanz based on the condition for which it is prescribed. Due to an increased risk of pulmonary embolism in RA patients, the FDA has warned patients to seek medical attention if they experience symptoms of a blood clot in the lungs.
Xeljanz Side Effects and Complications
The most common side effects of Xeljanz are:
- Upper respiratory infections;
- Headaches;
- Diarrhea; and
- Cold symptoms.
Serious side effects of Xeljanz include:
- Fatigue;
- Loss of appetite;
- Jaundice;
- Vomiting;
- Rashes;
- Fever and chills;
- Abnormal bowel habits;
- Chest pain;
- Shortness of breath;
- Cough accompanied by blood;
- Swelling and redness of the skin;
- Blood clots in the lungs (pulmonary embolism); and
- Death.
